Please note that the practice does not recommend providing letters of support. We find that these letters very rarely make a difference and as they are non-NHS work they will have a charge. We recommend that patients who seek to provide medical information to support their housing application do this using their own medical record, which they can obtain for free.
Our recommendation is to ensure you are set up on the free NHS App, which will allow you full access to your medical record and you can print all the relevant parts from your record. Once you've registered with Patient Access (free), click the pink bar below so we can link you to your medical record. Should you not be able to do this, you can request a print out of your medical records by clicking on this link.
